Dear Parents or Guardian, 

 

This coming September your child will be a senior in high school.   

 

All 12^th graders are required to be vaccinated for meningitis meningococcal (menACWY); one dose only if the 1^st dose was received at age 16+ or older.  Some students may need a third dose as the first 2 doses were given before age of 16; proof of immunization will be needed and signed by a physician or Board of Health person responsible for giving your child these shots. 

 

If you have not already done so, records must be brought to Ms. Bullock in room 371 or emailed to abullock@schools.nyc.gov.   Failure to provide proof of immunization could result in your child being excluded from school.  Please include student name and osis number on all medical records.
